Note to our Residents, Family, and Friends:

Greetings Family Members & Friends,

Park Central Care & Rehab is pleased to announce the indoor room/patio visitations we can offer. This is in addition to our outdoor, window/glass, video, and phone visits that we continue to provide for your on-going contact with loved ones/friends during the national emergency pandemic we are experiencing.

​

Appointments can be made Monday to Friday, 10:00AM to 4:30PM with our Social Services Director, Angel Sewak, or her designee, at (510) 797-5300 extension 207 for indoor or patio visits between the hours of 10:00AM to 4:00PM.

 

The facility Family room and it’s adjoining patio are the community settings for those visiting our “Green” zone, while a separate room is utilized for our “Yellow” zone observation area. Appropriate protective equipment shall be utilized after lobby screening for the respective zones. Full vaccination is required for indoor visitors, but unvaccinated may visit outdoors with a negative COVID-19 test within 24 hours.

 

If the visitor requires an antigen rapid test for COVID-19 at the time of visit, please arrive fifteen (15) minutes prior to your visit time and notify the facility of this request when making the appointment. The facility is not responsible for any adverse effects due to this requested testing.

A maximum of two (2) persons may visit per resident. The visitors are required to adhere to monitored visiting protocols. Non-compliance may result in exclusion. Length of visitations is thirty (30) minutes. Visitor traffic can only be to and from the designated communal spaces. Any children are counted as a person and must be able to follow the guidelines set forth for the adults. If a visitor has been in contact with a positive case, the visit must be rescheduled. The facility will apprise you and your family member/friend of any guideline changes.

Your help in making this process successful would be appreciated, including any needed rescheduling or changes, that our residents, your loved ones, can benefit from the normalcy of visitor contact, which we have made significant strides toward during this problematic time.

​

Due to Covid-19 Pandemic and restrictions placed by the Center for Diseases Control and State & Local Departments of Public Health, Park Central Care and Rehab continues to follow required guidelines for indoor visits by appointment only, except compassionate care/end of life situations, due to special circumstances.

​

We are all in this together. Let us all stay safe by wearing masks, practicing social distancing of 6 feet (if not fully vaccinated), washing and sanitizing hands, and refraining from any social gathering (if not fully vaccinated).  Thank you all for your understanding. Our staff is working hard to take care of your loved ones. Further updates will be posted as it becomes available. We thank all of you for your patience and sacrifices during this pandemic. 

 

If you want to visit your loved ones from outside through the window, we can open the drapes for you to see them and talk to them on the phone. Please schedule a time with facility staff. Virtual visits can be prescheduled and arranged through social services.

We hope this year you will make the holidays special for everyone the best way you can without in person contact.

Dear Residents, Family, and Friends: 

Due to the evolving COVID-19 (Coronavirus) Pandemic, the following guidelines will be implemented effective immediately, March 14, 2020 in accordance with the Centers for Medicare and Medicaid Services (CMS), Centers for Disease Control (CDC), and the local Public Health Department recommendations for the safety of our residents and healthcare workers.

 

  • ALL NON-ESSENTIAL VISITATION (VISITORS) WILL END UNTIL FURTHER NOTICE

  • RESTRICT RESIDENTS GOING OFF CAMPUS, EXCEPT FOR MEDICAL TREATMENTS 

​

  • CARE CONFERENCE MEETINGS WILL BE AVAILABLE VIA TELECOMMUNICATIONS 

​

  • WE ENCOURAGE FAMILY OR FRIENDS TO COMMUNICATE WITH RESIDENTS VIA TELECOMMUNICATIONS
